Aim of this study is to estimate the present and future number of MONW patients using data of random samples from a community-living population.\nMethods: Subjects were the seventh survey participants in the “National Institute of Longevity Center - Longitudinal Study of Aging (NILS-LSA)” a population-based prospective cohort study with age- and sex-stratified random samples. Prevalence of MONW with non-obesity with BMI and waist less than the reference value, and two or more metabolic abnormality with hyperglycemia, dyslipidemia, and hypertension by sex and age group was calculated, and the present and future number of patients in Japan was estimated using the characteristics of a representative cohort.\nResults: The prevalence of MONW in Japanese aged over 40 years old was estimated to be 10.9% for males and 13.6% for females, with 3.80 million men, 5.34 million women, and 9.14 million patients in total. And the number of patients will increase to 10.14 million in 2025 and 10.42 million in 2035 by the future estimation of the number of patients.\nConclusion: The estimated number of patients with MONW was almost equal to the estimated number of patients with metabolic syndrome in Japanese.
