{"created":"2023-06-20T12:59:41.185324+00:00","id":1598,"links":{},"metadata":{"_buckets":{"deposit":"3bfe0ce7-b8c7-4515-957c-679349f05c5c"},"_deposit":{"created_by":17,"id":"1598","owners":[17],"pid":{"revision_id":0,"type":"depid","value":"1598"},"status":"published"},"_oai":{"id":"oai:nufs-nuas.repo.nii.ac.jp:00001598","sets":["1:9:97:184"]},"author_link":["839","838","303","547","545"],"item_10002_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2019-12-25","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"5","bibliographicPageEnd":"44","bibliographicPageStart":"31","bibliographic_titles":[{"bibliographic_title":"名古屋栄養科学雑誌"},{"bibliographic_title":"Nagoya Journal of Nutritional Sciences","bibliographic_titleLang":"en"}]}]},"item_10002_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【目的】慢性血液透析患者の平均年齢は2017年末で68.4歳と高齢化しており、血液透析患者におけるフレイルの頻度は一般集団、保存期chronic kidney disease;CKD 患者よりも高率であると報告されている。また、低栄養状態とフレイルの関連も報告されているが、血液透析患者におけるフレイルと栄養状態との関係についての知見は少ない。そこで、フレイルの有症率と管理栄養士が用いることが多い栄養指標を使用し、フレイルと栄養状態との関連を明らかにすることを目的に調査を行った。\n【方法】透析医療機関に通院する血液透析患者298名を対象に患者背景因子、栄養評価、食事調査、血液生化学検査、身体計測を行い、Japanese version of the Cardiovascular Health Study( J-CHS)基準に基づきフレイルの判定を実施しその関連について比較検討を行った。\n【結果】対象者の平均年齢は69歳、平均透析歴110か月であり、ロバスト(健常)は67人(25.1%)、プレフレイルは147人 (55.1%)、フレイルは53人 (19.9%)であった。フレイルのphenotype が進行するごとに有意に年齢は上昇し、体重は少なく、四肢周囲径は低値を示した (P < 0.001)。栄養状態の指標である Geriatric nutritional risk index( GNRI)と mini nutritional assessment-short form( MNASF)において、有意に低値を示し( P = 0.046, P < 0.001)、Protein-energy wasting( PEW) 有症率はフレイルのphenotype の進行に伴い有意に増加していた (P < 0.001)。フレイルに関連する要因を検討したところ、年齢 (OR = 3.651, 95%CI 1.852-6.239, P = 0.003)、糖尿病 (OR = 3.056, 95%CI 1.526-5.359, P = 0.012)、BMI < 18.5( OR = 2.456, 95%CI 1.178-4.625, P = 0.022)、MNA-SF<11( OR = 4.202, 95%CI2.025-7.852, P < 0.001)、内服薬数 (OR = 3.247, 95%CI 1.254-4.653, P = 0.018)、転倒歴 (OR = 4.351, 95%CI 2.205-7.258, P = 0.007)、介護認定状況 (OR = 3.254, 95%CI 1.422-5.479, P = 0.018) が有意に関連していた。\n【結論】血液透析患者において、フレイルは高頻度に認め、低栄養に関連していた。また、MNA-SFはフレイルの簡便な栄養スクリーニング法であると考えられた。","subitem_description_type":"Abstract"},{"subitem_description":"[Purpose] Elderly patients undergoing hemodialysis are increasing, it is reported that the frequency of frailty in hemodialysis patients is higher than that in the general population, in chronic kidney disease (CKD) patients not on dialysis. In addition, the relationship between malnutrition and frailty has been reported, but there is little knowledge about the relationship between frailty and nutrition in hemodialysis patients. The purpose of this study was to examine the relationship between frailty and nutritional status.\n[Methods] In this study 298 hemodialysis patients attending dialysis medical institutions, background factors, nutritional assessment, dietary survey, was received blood biochemistry, and body measurements. Frailty phenotype was defined by the Japanese version of Cardiovascular Health Study (J-CHS) criteria, and the relation was compared.\n[Results] The subjects of this study were 69 years of age and dialysis history110 months. The subjects were 67 robust (25.1%), 147 pre-frailty (55.1%), and 53 frailty (19.9%). As the phenotype of the frailty progressed, the age increased, the low body weight, and the circumference of decreased (P < 0.001). Nutritional status showed a significant difference between Geriatric nutritional risk index (GNRI) and mini nutritional assessment-short form (MNA-SF) (P = 0.046, P < 0.001). In addition, the prevalence of Protein-energy wasting (PEW) increased as the phenotype of the frailty progressed (P < 0.001). nursing care (OR = 4.351, 95% CI 2.205–7.258, P = 0.007).\n[Conclusion] Frailty was frequently observed in hemodialysis patients and was associated with malnutrition.\nMNA-SF may be a simple nutritional screening method for
